In order to avail your Philhealth benefits, the following requirements must be submitted to Philhealth Office 24 hours proir to discharge.

Individually Paying Member/Self Employed
  • MDR (if there is)or PHILHEALTH No. only
  • Payment of contribution/ Official Receipt.
  • Those who dont have MDR, just proceed to Philhealthsection, upper ground for assessment.
    • Private/ Goverment Employed
  • Certificate of Contribution.
  • ER2

  • MDR with effectivity period
  • Certificate of eligibility form
  • 4P's I.D
    		•
    Overseas Filipino Workers (OFW's)
  • MDR with effectivity period
  • Payment of contribution/ Official Receipt.

    Lifetime Member/ Non paying Member
  • MDR pr Philhealth ID.

    RA10645 - Senior who don't have existing Philhealth Number
  • Senior Citizen ID.

    UNDECLARED DEPENDENTS

    Spouse:
  • Marriage Certificate with Registry Number + PMRF

  • Birth Certificate + PMRF
